Claim Services

At Madison National Life, we strive to make the claim process easy for you. Our experienced claim staff will work with you to direct the claim handling procedure as efficiently as possible. We are dedicated to providing our customers with claims services that are prompt, fair and supportive.

Reporting Your Claim

Reporting the claim allows us to begin the claim process quickly. You may do so by selecting the appropriate option from the choices listed below.

I have coverage through my employer or I am the employer (group life and disability)

I have an individual policy through the military, federal and postal services; through a small business payroll deduction plan; or was formerly with First National Life Insurance Company of America

I have an individual policy formerly with Southland National Insurance Corporation

I have an individual life, disability, annuity, final expense (pre-need) or any other types of individual coverage other than the above
