Name Change
You may correct and/or change your name by following one of the options below: (Please note: if your name is misspelled on your application, please include a copy of your birth certificate.)
- Call our Customer Service Department at 1-877-384-8511 Monday-Thursday 7am to 4:30pm, Friday 7am to Noon (CST) or use our secured Online Service Center; or
- Written notification to Madison National Life, PO Box 55975, Burmingham, AL 35255; or
- Download the Request for Service Form. Complete Section #2 of the Request for Service Form, sign, have witnessed and return to our office at the address listed above
