Changes and Requests

You may change the beneficiary be following one of the options below:

Call our Customer Service Department at 1-877-455-4300 Monday-Thursday 7am to 4:30pm, Friday 7am to Noon (CST) or use our secured Online Service Center; or
Written notification to Madison National Life, PO Box 55975, Burmingham, AL 35255; or
Download the Request for Service Form. Complete Section #4 of the Request for Service Form, sign, have witnessed and return to our office at the address listed above. (Please note: The owner must sign the forms. If you are the Power of Attorney or the Trustee, you must send a copy of the appropriate forms indicating such, if we do not have these records on file.)
