Group Medical

3 different HMOs to choose from or a Health Savings Account

Employer Contribution: MNL pays 100% of the monthly premium for each employee’s coverage and contributes %90 per month toward family coverage.

Effective Date: 1st of the month following 30 days

Group Dental

PPO plan provided.
$50 annual deductible and $2000 calendar year maximum benefit

Employer Contribution: MNL contributes up to $45.00 towards each employee’s monthly premium, which covers the employee only coverage and offsets the expense of dependent categories.

Section 125 Flexible Spending Account

Unreimbursed Medical Spending Account --$2500 maximum annual election
